From Prescription to Participation:
Building an Integrated Preventive Health Ecosystem

The success of preventive healthcare is increasingly shaped not only by what happens within hospitals and clinics, but by what happens when individuals return to their homes and communities. Turning clinical advice into sustained action requires stronger connections between healthcare, community resources, behavioural support, accessible programmes and technology.

But how do we make that connection work in practice?

Led by Dr Fadzil Hamzah, Senior Staff Physician, Department of Sport and Exercise Medicine at Changi General Hospital, this multidisciplinary conversation brings together perspectives from across Singapore’s preventive health ecosystem to explore the journey from prescription to participation.

Drawing on real-world experience, the discussion will examine how individuals can be identified and supported earlier, how clinical recommendations can be connected with appropriate community programmes and resources, and what helps people translate advice into meaningful and sustainable changes in their everyday lives.

The conversation will also look beyond individual interventions to the wider ecosystem required to make preventive health work: how healthcare and community partners can collaborate across traditional boundaries, where gaps remain, what enables sustained participation and behaviour change, and how technology can support more connected and person-centred models of care.

How can we better bridge the transition from clinical recommendation to community participation?

What helps individuals adopt and sustain healthier behaviours beyond the clinical setting?

How can healthcare providers, community organisations and other partners work more effectively together?

Where are the gaps between healthcare and community support, and what needs to change?

How can technology strengthen access, engagement and continuity of care without losing the human element?

What can we learn from real-world programmes and experiences that could be applied across different healthcare and community settings?

For healthcare and rehabilitation practitioners, community-care professionals, policymakers and other stakeholders from Singapore and across the region, the session offers practical perspectives and transferable lessons for strengthening the connections between clinical care and everyday life, improving health outcomes, supporting healthy ageing and enabling people to remain active and well within their communities.

Dr Fadzil Hamzah​

Senior Staff Physician, Department of Sport and Exercise Medicine | Changi General Hospital

Dr Fadzil Hamzah is a Senior Staff Physician in the Department of Sport and Exercise Medicine at Changi General Hospital and Operational Lead of the Singapore Sport and Exercise Medicine Centre at Singapore General Hospital. He also serves as Deputy Director, Health Promotion and Disease Prevention at the SingHealth Office of Regional Health; Director (Community) at the SingHealth Duke-NUS Sport and Exercise Medicine Centre; and Director (Community Programmes) of Exercise is Medicine Singapore.

His professional interests span sport and exercise medicine, preventive health, physical activity promotion and population health. Beyond clinical practice, he develops community-based health initiatives and multidisciplinary partnerships across healthcare institutions, government agencies and community organisations. He is passionate about translating evidence into practical interventions, promoting physical activity as medicine, and strengthening the bridge between healthcare and community. His work focuses on empowering individuals and communities to prevent and better manage chronic diseases through sustainable lifestyle change, community participation and integrated preventive care.”

Genevieve GOH

Director, Active Health, Sport Development Group | Sport Singapore

Ms. Genevieve Goh is the Director of the Active Health division at Sport Singapore (SportSG). She leads Active Health – a national initiative by SportSG launched to encourage individuals in Singapore to proactively manage their health, adopt sustainable fitness habits, and integrate physical activity into their daily lives. Her role involves shaping health and wellness strategies, operating a network of Active Health Labs across the island, collaborating with medical and community partners (such as the Health Promotion Board, Agency for Integrated Care and various healthcare clusters), and promoting preventive health through sports and exercise science.

Todd Crane

Health, Corporate & Performance KA Manager APAC

Todd Crane is the Regional Account Manager for Technogym, a leader in fitness equipment known as “The Wellness Brand”. He oversees Health, Corporate, and Performance accounts in the APAC region, that includes a significant focus on active ageing initiatives. Originally from Australia, Todd has a strong background in sports medicine and rehabilitation products, with a passion for promoting a wellness mindset centred on physical activity and positive mental health. He states, “I’m proud to work for Technogym, an innovative organization that helps people live longer, happier, and healthier lives through top-quality equipment, content, and a unique digital ecosystem.”

Pearline Lee

Deputy Director, Health and Social Integration | SingHealth Office of Regional Health (SORH)

Ms Pearline Lee is the Deputy Director of Health & Social Integration (HSI) at the SingHealth Office of Regional Health (SORH). She leads the harmonization and development of Wellbeing Coordinators across SingHealth institutions, working closely with internal and external stakeholders to drive health and social care integration.

Ms Lee was instrumental in the establishment and development of CareLine, a 24-hour personal telecare service that supports seniors in the community. Since its inception in 2016, CareLine has built a strong ecosystem of healthcare, community and social service partners to ensure timely care, assistance and referrals for seniors in need.

Under her leadership, CareLine has expanded its role beyond telecare to become the response provider for the wireless Alert Alarm System (wAAS), delivering round-the-clock emergency response support for vulnerable seniors aged 60 and above living in public rental housing.

Nanny Eliana (Nan)

Bridges M&C Medical & Healthcare Communications Agency

Nan is a healthcare communicator with more than 20 years’ experience working across Southeast Asia and the wider Asia-Pacific with pharmaceutical and MedTech companies, public health institutions, and patient advocacy groups. She has extensive experience engaging healthcare leaders, including policymakers, clinicians, and industry stakeholders, in unpacking complex healthcare and clinical issues and examining what they mean in the real world for different audiences.
